Camper Physicals

Yearly physicals need to be added on the camp registration system prior to the camper’s visit.  Optionally, you can mail the form to: Attn: Nursing, Camp Courageous, PO Box 418, Monticello, IA 52310. Physicals can be scanned and E-mailed to nursing@campcourageous.org, or fax the form to 319-465-5919.
